- To process volunteer, board, and program interest submissions.

Please do not submit clinical information, diagnoses, medication information, insurance information, Social Security numbers, or protected health information (PHI) through our public forms. We do not knowingly collect PHI through this website.
Sharing
We do not sell your personal information. We share limited information only with service providers (for example, Stripe for payment processing and email delivery vendors) as needed to operate the Foundation.

Children
This site is not directed to children under 13. We do not knowingly collect information from children under 13.
